How to access our service


Call our Positive Living Outreach Line on 01382 561822. If we’re out and about when you call, you’ll be asked to leave a message and we will get back to you the next working day.


Alternatively, email Referral details should include name, contact details and relevant background information to explain why the individual would benefit from our service. When referring someone else, please get their consent to share their details with us.


To allow staff to conduct the necessary risk assessment, please include in the referral if there are any known concerns about the individual being visited alone.


We are a responsive service and aim to make contact with individuals within 24 hours of referral (for referrals received Monday-Thursday). This service does not operate over the weekend, and referrals received Friday-Sunday will be responded to on the Monday.
